Medication Error
There are a number of different types of error that can happen when medication is prescribed or administered in a healthcare situation.
In the main, these are the same across all areas of healthcare provision. However, the ones that we come across most often that relate to Mental Health care are:
- No justification for administration of a particular drug, particularly anti-psychotic medication
- Ongoing prescription of addictive medicines such as diazepam
- Prescription of an incorrect medication regime
- Prescription of an incorrect dosage of a drug
